Cosmetology Licensure Renewal
When your license is near its expiry state date, a renewal application form is sent to your mail. So ensure that you input the right biodata when you initialing apply, and then update the board in the case of any change. I’ll be addressing how to do that below. Now, if you do not receive the form on by the last day of your license, do not panic. There’s a way around that. Simply send a note requesting the renewal of your license including an additional $5 fee.
You may pay by cashier’s check or a money order, but not a personal or temporary check. Ensure your check includes the right name, mailing address, and social security number.
The address to use is:
- Oklahoma State Board of Cosmetology and Barbering
- 2401 NW 23rd, Suite 84
- Oklahoma City, OK 73107
Initial license fee: $25
Renewal fee (to be paid every year): $25
Late renewal fee: $10
Once your license is two months past expiration, you’ll have to pay an additional late fee to renew your license.
How to transfer my Cosmetology license to Oklahoma from another State?
Considering a move to a more friendly neighbourhood in the Sooner’s state? Good news, you can take your practice along with you. Oklahoma State Board of Cosmetology and Barbering even allows reciprocity without an examination. However, you will have to pass the Board’s Sanitation and Safety Examination.
You’ll first have to complete and submit a reciprocity application form, as well as a notarized Affidavit verifying you are in America lawfully. You’ll also need to send in a full face photograph and a cashier’s check for $65 with your form. $5 is for Oklahoma’s Rule and Regulation book, while the other $60 is for your license.
Of course your home state will need to send down a copy of your current certification and license records. Once Oklahoma’s boardhas received this information, a representative will update you. Keep in mind your records will contain your total working history, and number of training hours completed. If your application is accepted, you will receive a notification with a scheduled date and time for your Sanitation and Safety Exam. On passing, you will receive your license.
How to change name/address on Cosmetology license
Name or address changes aren’t unusual, so it’s only natural the board would make provisions for such updates. So for official purposes, and to keep your records current, inform the Cosmetology board of any important change as soon as possible. For a name change, you’ll have to submit a legal document (such as a court declaration, divorce decree, marriage certificate, or even an update driver’s license). Following that, you can go ahead and request a new copy of the license with your new name. Of course you’ll have to include a token fee of $5.
If you wish to change your address on the other hand, you’ll need to send a location change affidavit.
